Day shift, 5x8 hours (08:00 AM - 4:30 PM); this facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a skilled Home Health Physical Therapist to join our team in Medford, OR. The Physical Therapist will provide one-on-one care to patients in their homes, with a focus on rehabilitation and recovery.

Responsibilities

• Conduct evaluations and develop personalized treatment plans for patients.
• Provide direct patient care, including therapy interventions and education on home exercise programs.
• Accurately document patient progress using OASIS on the EPIC charting system.
• Manage a caseload of up to 6 patients per day, ensuring high-quality care and adherence to productivity points.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience is required.
• BCLS certification is required.
• OASIS experience is mandatory; lack of experience may lead to disqualification.
• Traveler must have reliable transportation to visit patients.
• COVID vaccination primary series and one booster required.
