Job Description

The Home Health Physical Therapist will provide skilled care to patients in their homes in West Portland and surrounding areas. The role requires strong clinical skills, effective communication, and the ability to manage a caseload efficiently while adhering to productivity standards.

Responsibilities

• Conduct evaluations and develop individualized treatment plans for patients.
• Provide therapeutic interventions to assist patients in achieving their rehabilitation goals.
• Document patient progress in accordance with agency policies and regulatory requirements, including OASIS documentation.
• Work collaboratively with healthcare teams to optimize patient care.
• Manage a caseload, which may include visiting up to 6 patients per day, ensuring adherence to productivity standards.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience.
• Current Oregon Physical Therapist license.
• Basic Life Support (BCLS) certification.
• OASIS experience is required.
• Reliable transportation is a must, with occasional travel expected up to 60 miles.
• COVID vaccination series and one booster is required.
