Job Description

We are seeking a skilled Home Health Physical Therapist to provide exceptional patient care in various settings. The ideal candidate will be responsible for conducting assessments, developing home exercise programs, and educating patients and their families on safety and injury prevention while adhering to professional standards.

Responsibilities

• Conduct patient assessments and develop individualized treatment plans.
• Perform evaluations and document patient progress accurately.
• Utilize various equipment and assistive devices for patient rehabilitation.
• Educate patients and families about care plans, safety, and rehabilitation strategies.
• Monitor patient vitals and report changes to the healthcare team.
• Administer medications as prescribed and adhere to all safety protocols.

Required Experience / Certifications / Licensure

• Minimum of 1 year of Home Health experience required.
• Active Physical Therapist license in the state of Florida or a compact license.
• BLS certification required.
• OASIS training preferred.
• Must possess a valid driver's license and have reliable transportation for home visits.
