Physical Therapist - Home Health

This is a 13-week contract position.

Schedule

Day shift, 5 days a week from 08:00 AM to 05:00 PM.

Job Description

The Physical Therapist will provide exceptional care to patients in a home health setting. The role includes developing and implementing treatment plans, conducting patient evaluations, and collaborating with interdisciplinary teams to enhance patient outcomes.

Responsibilities

• Conduct assessments and evaluations of patients' physical abilities.
• Create and implement individualized treatment plans.
• Document patient progress and outcomes in accordance with established guidelines.
• Work collaboratively with healthcare professionals to coordinate patient care.
• Educate patients and their families about rehabilitation processes and home exercise programs.

Required Experience / Certifications / Licensure

• Graduate degree from an accredited Physical Therapy Program (approved by the APTA).
• Minimum of one year of physical therapy experience, home health or facility preferred.
• Current and unrestricted Physical Therapy Licensure.
• Current CPR certification.
