Day Shift: 5x8-Hour (08:00 - 16:30)

Job Description

We are seeking a passionate and experienced Home Health Physical Therapist to deliver high-quality care to patients in their homes. The therapist will work independently and must have strong experience in home health settings, specifically with OASIS documentation.

Responsibilities

• Provide PT services for a caseload of patients, managing up to 6 patients per day.
• Perform evaluations, develop treatment plans, and implement therapy interventions tailored to individual patient needs.
• Conduct OASIS assessments and maintain accurate patient records using EPIC/Oasis charting systems.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Educate and support patients and their families on treatment plans and home exercise programs.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience is required.
• Current BCLS certification.
• Experience with OASIS documentation is mandatory.
• Reliable transportation is necessary due to patient visit logistics.
• Active PT license valid in Oregon is required.
