Job Description

We are seeking an experienced Home Health Physical Therapist to provide individualized therapy services to patients in their homes. The ideal candidate will have a passion for helping patients improve their mobility and overall well-being.

Responsibilities

• Conduct evaluations and develop individualized treatment plans for patients.
• Utilize OASIS for patient assessments and documentation.
• Implement therapeutic interventions tailored to patient needs.
• Communicate effectively with patients, families, and healthcare team members.
• Ensure compliance with all regulatory requirements and agency policies.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled Home Health experience required.
• BCLS certification required.
• OASIS experience required.
• Active state Physical Therapist license.
