Day Shift: 5x8-Hour (08:00 - 16:30)

Job Description

The Physical Therapist will provide skilled home health services, focusing on therapeutic interventions for patients in their homes. The role involves evaluating patient progress, creating and implementing treatment plans, and working closely with patients to achieve rehab goals. The therapist will utilize OASIS for documentation while seeing up to 6 patients per day.

Responsibilities

• Conduct patient evaluations and assessments.
• Develop and implement tailored treatment plans.
• Monitor patient progress and adjust treatment plans as necessary.
• Document patient care and progress in compliance with OASIS standards.
• Educate patients and families on rehabilitation programs and home exercises.
• Provide timely and efficient home health services while minimizing travel time.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience.
• Current OR Physical Therapist license.
• Basic Life Support (BCLS) certification.
• OASIS experience is required.
• Reliable transportation required for patient visits.
