Job Description

The Home Health Physical Therapist Assistant will provide effective treatment plans to patients in their homes, assisting with their rehabilitation needs and facilitating recovery. The professional will manage up to six patient visits per day with a 1:1 care ratio, ensuring high productivity and quality of care.

Responsibilities

• Execute therapy interventions as per established treatment plans.
• Monitor patient progress and document services provided using the EPIC Remote Client for Home Health.
• Utilize appropriate techniques and specialized equipment in patient care.
• Communicate effectively with patients, families, and other health care professionals regarding patient needs and progress.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• BCLS certification is required.
• Reliable transportation is a must, with occasional patient visits requiring up to 60 miles of travel in a day.
• Primary series COVID-19 vaccination plus one booster is required.
