Day shift, 5x8-hour schedule (08:00 AM - 04:30 PM).

Job Description

We are seeking a dedicated and experienced Home Health Physical Therapist to provide exceptional care to patients in their home environment. As a Physical Therapist, you will have the opportunity to evaluate and treat patients, helping them restore function and improve their quality of life.

Responsibilities

• Assess patients' physical abilities and evaluate their needs.
• Develop individualized treatment plans and goals for each patient.
• Provide skilled therapeutic interventions to facilitate recovery.
• Utilize OASIS for documentation and patient care coordination.
• Educate patients and their families on treatment plans and necessary home exercises.
• Maintain accurate patient records and documentation in electronic systems.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• Current licensure as a Physical Therapist in Oregon or Washington.
• Basic Cardiac Life Support (BCLS) certification required.
• Must have experience with OASIS documentation.
• Reliable transportation required for travel between patient homes.
