Occupational Therapist (OT) - Outpatient and Home Health

Monday - Friday; variable hours

Job Description

This position is responsible for enhancing, participating, and supporting patient care with a focus on quality and safety. The Occupational Therapist (OT), with a physician referral, is authorized to treat patients in accordance with O.T. Practice Guidelines as issued by the American Occupational Therapy Association. The O.T. will work in an interdisciplinary manner with other departments in order to provide quality care. Must demonstrate proficiency and proper utilization of the electronic medical record, including proper documentation. This position is primarily outpatient and home health with occasional inpatient coverage.

Responsibilities

• Provide evaluations, treatment plans, and therapy interventions for patients.
• Collaborate with interdisciplinary teams to ensure effective patient care.
• Utilize electronic medical records accurately for documentation.
• Deliver care in both outpatient settings and home health environments.

Required Experience / Certifications / Licensure

• Valid Occupational Therapy license.
• Experience in outpatient and home health settings preferred.
