Registered Nurse Non-Certified Case Manager

Day Shift: 7:30 AM - 4:30 PM, Weekend Rotations

Job Description

The Registered Nurse Non-Certified Case Manager collaborates with physicians and multidisciplinary team members to develop care plans for assigned patients. This role is vital in monitoring patient progress through assessments and patient records while identifying and resolving barriers to effective patient care. The RN will also actively participate in the discharge planning process.

Responsibilities

• Conduct comprehensive assessments of members per regulated timelines.
• Develop and implement case management plans in collaboration with members, caregivers, physicians, and other healthcare professionals.
• Perform ongoing monitoring of care plans to evaluate effectiveness and document interventions.
• Maintain an ongoing member case load for regular outreach and management.
• Promote integration of services for members, including behavioral health care and long-term services.

Required Experience / Certifications / Licensure

• Active Montana Licensure as a Registered Nurse.
• American Heart Association BLS Certification.
• Minimum of 2 years of nursing experience with a focus on utilization review.
• Preferred: Bachelor’s Degree in Nursing and 2 years of acute care hospital experience as a case manager.

Assignment Duration

This is a 13-week contract position.
