Physical Therapist - Home Health

This is a 13-week contract position.

Schedule

Day shift: 5x8-Hour (08:00 - 17:00).

Job Description

The Physical Therapist will provide comprehensive therapy services to patients in a home health setting. Utilizing clinical expertise, the therapist will evaluate patient needs, develop treatment plans, and implement therapeutic techniques to improve mobility and quality of life.

Responsibilities

• Conduct initial evaluations and develop individualized treatment plans.
• Implement therapy interventions to enhance patient function and mobility.
• Collaborate with interdisciplinary teams to optimize patient care.
• Document patient progress and adjust treatment plans as necessary.
• Educate patients and their families regarding rehabilitation strategies.

Required Experience / Certifications / Licensure

• Graduate degree from an accredited Physical Therapy Program (approved by the APTA).
• Minimum of one-year experience in physical therapy, preferably in home health or facility settings.
• Current and unrestricted Physical Therapy Licensure.
• Current CPR certification.
