Job Description

We are seeking a dedicated Home Health Physical Therapy Assistant (PTA) to provide care and support to patients in their home. The PTA will work closely with Physical Therapists to implement treatment plans, assist in evaluations, and help patients achieve their therapeutic goals.

Responsibilities

• Assist in implementing treatment plans as directed by the supervising Physical Therapist.
• Perform therapy interventions in various home settings based on patient needs.
• Document patient progress and report any changes in condition to the therapist.
• Conduct an average of 6-7 visits per day, depending on the nature of the visits.
• Collaborate with team members to ensure comprehensive care for patients.
• May be asked to assist in other territories within a 60-mile radius of the main location as needed.

Required Experience / Certifications / Licensure

• Prior experience in home health care as a Physical Therapy Assistant is required.
• Homecare Homebase documentation experience is required; candidates without this experience will be considered at a reduced rate for the first two weeks if willing to undergo training.
• Must possess current PTA licensure or certification as mandated by state regulations.
