LPN - Full Time Nights

Full time nights, with every other weekend and holiday required.

Job Description

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit and maintain a collaborative relationship with nursing and ancillary staff.

Responsibilities

• Collect data relevant to resident's physical and psychosocial condition and communicate findings to the RN and other team members.
• Develop age-appropriate and culturally relevant care plans in collaboration with residents, families, and healthcare teams.
• Evaluate resident outcomes and modify care plans as needed to ensure comprehensive care.
• Participate in the mentoring and orientation of staff, promoting a safe and efficient care environment.
• Maintain current knowledge and competency in nursing practice through continuing education.
• Ensures safety and compliance with protocols addressing blood-borne pathogens.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification (AHA or RQI only)
• Minimum of 1 year experience as an LPN in a Long-Term Care Facility
