Part-Time Licensed Practical Nurse (LPN)

Job Description

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide care needed by adult and geriatric patient groups.

Responsibilities

• Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicate findings to RN and other members of the Interdisciplinary Team.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired outcomes.
• Recommend revisions to the plan of care and collaborate with the resident/family and members of the Interdisciplinary Team.
• Maintain current knowledge and competency in Nursing Practice; participate in the mentoring/orientation of staff.
• Demonstrate leadership in the professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification (AHA or RQI specifically)
• 1 year of experience serving in an LPN capacity at a Long-Term Care Facility

Schedule

This position requires weekend and holiday availability, with rotational shifts of 2 or 3 days per week.

Assignment Duration

This is a 13-week contract position.
