Rehabilitation Therapist - Skilled Nursing Facility

Job Description

We are seeking a dedicated Rehabilitation Therapist to provide rehabilitation services in a Skilled Nursing Facility (SNF) or Intermediate Care Facility (ICF). This role involves collaborating with an interdisciplinary team to develop and implement individualized treatment plans, ensuring the highest quality of care for our residents.

Responsibilities

• Perform evaluations, reassessments, and treatments consistent with licensure, discipline, and scope of practice.
• Develop and implement individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team.
• Implement evidence-based therapeutic interventions to maximize function, independence, communication, mobility, swallowing, and quality of life.
• Educate residents, families, and caregivers about treatment plans and patient progress.
• Complete accurate, timely, and compliant documentation in accordance with facility, state, federal, and payer requirements.
• Supervise therapy assistants, when applicable to the role.
• Available to work on Hawaii State and Federal Holidays as necessary based on facility operations.

Required Experience / Certifications / Licensure

• Minimum of two years of experience in the SNF/ICF setting preferred.
• Current Basic Life Support (BLS) certification.
• Current State of Hawaii license in good standing appropriate to the discipline.

Schedule

Generally scheduled for five days per week, likely to include one weekend day. Flexible to float to other units or facilities based on census and staffing needs.

Assignment Duration

This is a 13-week contract position.
