Licensed Practical Nurse (LPN) - Long Term Care

Schedule

Shift: 2:30 PM - 10:30 PM (8-hour evening shift) - 37.5 guaranteed hours. Every other weekend (EOW) is required. Please note that block scheduling is not guaranteed and floating within the same building is required.

Job Description

The Licensed Practical Nurse (LPN) will provide nursing care to residents in a Long Term Care (LTC) setting, ensuring the highest levels of patient care and comfort. The LPN will work collaboratively with a team of healthcare professionals to meet the needs of residents with various diagnoses, including multiple sclerosis, orthopedic conditions, and dementia.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor and document patient vital signs and health changes.
• Assist residents with activities of daily living (ADLs).
• Communicate effectively with residents, families, and the healthcare team.
• Ensure a safe and clean environment for residents.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience in a Long Term Care facility.
• Current New York State LPN license is required.
• Verified current Basic Life Support (BLS) certification.
• Familiarity with PointClick Care electronic medical records is preferred.
• Drivers license with an address that matches demographics.

Assignment Duration

This is a 13-week contract position.
