Day shift, 5x8 hours (08:00 - 16:30). Note: This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a dedicated Home Health Certified Occupational Therapy Assistant (COTA) to provide personalized therapy services to patients in their homes. The ideal candidate will have a passion for enhancing patients’ quality of life through meaningful therapeutic interventions.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Implement therapeutic procedures in a home setting for clients with various conditions.
• Monitor patient progress and adjust treatments as needed to achieve desired outcomes.
• Collaborate with other healthcare professionals to coordinate care and improve patient outcomes.
• Educate patients and their families on therapeutic activities and exercises.
• Document patient care and maintain accurate records in an electronic system.

Required Experience / Certifications / Licensure

• Must hold a valid Oregon Certified Occupational Therapy Assistant license.
• Minimum of 1 year of skilled home health experience.
• Current Basic Life Support (BCLS) certification.
• Experience using OASIS and functional independence measurement tools preferred.
