Licensed Practical Nurse (LPN) - Long-Term Care

Job Description

This is a 13-week contract position for a Licensed Practical Nurse (LPN) in a Long-Term Care (LTC) facility. The ideal candidate will provide high-quality nursing care to patients with various diagnoses, including multiple sclerosis, orthopedic conditions, dementia, and those requiring memory care.

Schedule

Shift: 6:30 AM - 2:30 PM (8-hour shifts).

Responsibilities

• Deliver patient care in compliance with facility policies and procedures.
• Administer medications and monitor patient responses.
• Assist with activities of daily living (ADLs) and provide general support to residents.
• Document patient care using Electronic Medical Records (EMR) systems.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Participate in floating duties within the same building as required.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience in a Long-Term Care facility.
• Valid and active LPN license in New York (NURSYS report required).
• Current Basic Life Support (BLS) certification.
• Driver's license with an address matching demographics.
