Job Description

We are seeking a dedicated Home Health Physical Therapist to provide essential services to patients in their homes. The ideal candidate will have a strong background in rehabilitation, particularly with conditions related to orthopedics and prosthetics/orthotics.

Responsibilities

• Conduct skilled home visits to assess and treat patients.
• Implement individualized home exercise programs.
• Educate patients and families on safety, injury prevention, and rehabilitation strategies.
• Utilize evaluation tools and appropriate documentation for patient records.
• Collaborate with interdisciplinary teams to optimize patient care.

Required Experience / Certifications / Licensure

• 2 years of relevant experience in home health settings.
• Valid state license as a Physical Therapist in Florida or a compact state license.
• BLS certification.
• Experience with OASIS preferred.
• Ability to manage 6-8 patient visits per day.
• Must have a valid driver's license and a car for patient visits.
• COVID and flu vaccinations are required; exemptions must be documented.
