Registered Nurse - Hospice Triage & On-Call Visit

Schedule

5PM to 5AM with on-call from 5AM to 8AM, requiring availability for 36 hours per week.

Job Description

The Registered Nurse in this position will be responsible for triaging after-hours calls and collaborating with the inpatient Sussman House team. The role includes completing after-hours admissions and visits and conducting check-in calls with patients admitted between 8AM and 5PM.

Responsibilities

• Triage after-hours calls and provide collaboration with the inpatient team.
• Complete after-hours admissions and patient visits.
• Conduct check-in calls with patients admitted during regular business hours.
• Work collaboratively with the inpatient Sussman House team.

Required Experience / Certifications / Licensure

• Current RN licensure required.
• Experience in hospice care is preferred.
