Case Manager - Discharge Planning

This is a 13-week contract position.

Schedule

Shifts are Monday to Friday from 8:00 AM to 4:30 PM (7.5-hour shifts with a 1-hour lunch).

Job Description

The Case Manager in Discharge Planning is responsible for evaluating and facilitating patient transitions from inpatient settings. The role involves working closely with an interdisciplinary team to ensure appropriate discharge planning and referrals.

Responsibilities

• Collaborate with social workers and medical staff on discharge planning.
• Manage a caseload of 17-25 patients, ensuring timely and effective discharge processes.
• Round with physicians and the interdisciplinary team on the unit.
• Assist in securing necessary resources for patients post-discharge, including housing and community services.
• Maintain accurate documentation in the Allscripts EMR system.
• Respond to and manage patient needs in a timely manner.

Required Experience / Certifications / Licensure

• Minimum of 1 year of inpatient case management experience required.
• Prefer prior experience in critical care settings.
• Required Certifications: BLS Certification, PRI Certification, SCREEN Training Certification.
• Familiarity with InterQual criteria, CarePort, and MCG is preferred.
