5x8.0 schedule with flexible hours, including 1-2 evenings until 7 PM.

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on individual needs, goals, and physician direction. This role includes delegating and directing therapy services, ensuring functional treatment programs are designed for effective patient return to community activities. The therapist serves as an educator for patients, families, students, and coworkers while adhering to professional standards and organizational compliance.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document patient care accurately and timely in compliance with state and federal guidelines.
• Ensure care is provided under signed, valid physician orders within insurance approval.
• Participate in student program assignments, fostering a positive learning environment.
• Engage in continuous professional development and adhere to state practice acts and regulations.
• Contribute to organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active physical therapy license for the state of employment.
• Previous experience with the agency is highly desired.
