Day shift, 5x8-hour schedule (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a skilled Home Health Occupational Therapist to provide high-quality patient care in the West Portland area. The ideal candidate will have a passion for enhancing the quality of life for patients in a home setting, utilizing their expertise to facilitate recovery and rehabilitation.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans.
• Implement therapeutic interventions to enhance patient independence and quality of life.
• Document patient progress and adjust care plans as necessary.
• Educate patients and families about treatment plans and goals.
• Collaborate with healthcare professionals to ensure comprehensive care.
• Utilize OASIS for patient documentation as required.

Required Experience / Certifications / Licensure

• Must hold a valid Oregon Occupational Therapist license.
• Minimum of 1 year of skilled home health experience is required.
• BCLS certification is necessary.
• Experience with OASIS documentation is highly preferred.
• Reliable transportation is required for travel between patient visits.
