Job Description

The Occupational Therapist is responsible for screening, evaluating, and developing care plans for patients with cognitive and/or ADL physical functioning disorders. The role requires effective communication and collaboration with patients, families, caregivers, and other service providers.

Responsibilities

  • Screen and evaluate patients with cognitive and/or ADL physical functioning disorders.
  • Develop appropriate plans of care adhering to regulatory and clinical practice standards.
  • Utilize standardized assessments and evidence-based practice to support clinical interventions.
  • Provide rehabilitative, skilled, and medically necessary treatment interventions.
  • Administer various rehabilitation procedures including ADL retraining, therapeutic activities, and exercises.
  • Document all required information in the electronic documentation system and maintain patient records.
  • Assess patient progress and adjust treatments based on evaluation outcomes.
  • Provide consultation and counseling to patients, families, and caregivers.
  • Participate in patient care conferences, utilization review meetings, and required trainings.
  • Supervise occupational therapy assistants and rehabilitation aides or students.

Required Experience / Certifications / Licensure

  • 1+ years of Occupational Therapy experience (REQUIRED).
  • Experience in Long Term Care (LTC) or Rehabilitation (REQUIRED).
  • Active Occupational Therapy license (REQUIRED).
  • New graduates considered with Level II experience, not just Level I observation.
