AM shifts, Monday through Friday, totaling 40 hours a week.

Job Description

We are seeking a dedicated Home Health Physical Therapist to provide quality care to patients in their homes. The role involves conducting evaluations, developing treatment plans, and delivering therapeutic interventions to facilitate recovery and improve the quality of life for our clients.

Responsibilities

• Conduct evaluations and create individualized treatment plans.
• Deliver therapy services in accordance with established plans of care.
• Complete necessary documentation and OASIS assessments accurately.
• Coordinate with other healthcare professionals and participate in case conferences.
• Manage patient caseload while maintaining productivity standards.

Required Experience / Certifications / Licensure

• Experience in Home Health and Oasis documentation is required.
• Active Physical Therapist license is mandatory.
• Homecare Homebase documentation experience is preferred; however, candidates without this experience may be considered at a reduced rate if they are willing to undergo training.
