Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) for a position in Long Term Care, specifically on the Subacute Rehab floor. The ideal candidate will provide high-quality nursing care to patients with common diagnoses including dementia, chronic diseases, and diabetes.

Responsibilities

• Provide nursing care to patients in accordance with established care plans.
• Administer medications and assist with daily living activities.
• Monitor patient progress and document care in PointClickCare eMAR.
• Collaborate with healthcare team members to enhance patient outcomes.
• Support patients and families with education and emotional support.

Required Experience / Certifications / Licensure

• 1-3 years of experience required; SNF experience is acceptable.
• Active NY nursing license required.
• BLS certification (AHA) is required.
• Ability to work within a 100-mile radius rule.
