LPN - Long Term Care

This position offers a shift from 2:30 PM to 10:30 PM, with every other weekend required.

Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide quality care to patients in a long-term care setting. The ideal candidate will have experience working with diverse patient populations, including those with multiple sclerosis, orthopedic conditions, dementia, and memory care needs. This position requires flexibility to float to all units within the same building.

Responsibilities

• Provide nursing care to patients in accordance with established policies and procedures.
• Administer medications and treatment as prescribed.
• Assist with activities of daily living (ADLs) and provide support to patients and families.
• Maintain accurate patient records using the PointClick Care electronic medical record system.
• Communicate effectively with the healthcare team regarding patient status and needs.

Required Experience / Certifications / Licensure

• Must have 1+ years of experience in a similar role.
• Current New York State LPN license is required.
• BLS certification is required.
