Job Description

As an RN Case Manager at Visiting Nurse and Hospice for Vermont and New Hampshire, you will become an integral part of a cohesive patient-care team. You will have the opportunity to get to know your patients, providing them with the comprehensive care they need while maintaining a work/life balance that allows for flexibility.

Responsibilities

• Develop care plans in collaboration with the patient's physician, the patient, and their family or primary caregivers upon admission.
• Periodically review and recertify the patient care plan as appropriate.
• Coordinate and communicate the patient's care plan through interdisciplinary communication and case conferences.
• Advise other disciplines of discharges in a timely manner.
• Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and progress towards desired goals.
• Document evaluations, treatment goals, and objectives.
• Teach patients and caregivers self-care techniques and provide medication, diet, and other instructions as ordered by the physician.

Required Experience / Certifications / Licensure

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license.
• Graduate of an accredited school of nursing.
• 2 years of experience in Home Health.
