Licensed Practical Nurse (LPN) - Full Time D/E Rotation

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents, ensuring that the needs of adult and geriatric patients are met through comprehensive and compassionate care. The LPN must demonstrate the knowledge and skills necessary to provide age-appropriate care while collaborating with the interdisciplinary team.

Responsibilities

• Collect data relevant to residents' physical and psychosocial conditions on an ongoing basis and communicate findings to the Registered Nurse and other team members.
• Contribute to the development of culturally relevant care plans reflecting nursing diagnoses and interventions to achieve desired outcomes.
• Advise on revisions to care plans based on evaluations of resident outcomes and indicate the need for referrals to address all facets of care.
• Participate in mentoring and orientation of staff, while maintaining current knowledge and competency in nursing practice.
• Provide leadership in the professional setting, considering safety, cost, and impact in the planning and delivery of care.
• Ensure compliance with safety standards regarding occupational exposure to bloodborne pathogens.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification through the American Heart Association (AHA) or RQI
• Minimum of 1 year experience as an LPN in a long-term care facility
• Availability for holiday and evening shifts as required

Schedule

This position requires full-time availability for Day and Evening shifts, working 5x8-hour rotations.
