Licensed Practical Nurse (LPN) - Long Term Care

Job Description

An experienced Licensed Practical Nurse (LPN) is required for a position in a Long Term Care Facility. The role involves a variety of patient care responsibilities and administrative tasks.

Responsibilities

• Performing assessments and delivering treatments.
• Administering medications.
• Notifying the MD/NP of pertinent patient information.
• Assisting with admitting and discharging patients.
• Providing assistance with Activities of Daily Living (ADLs).
• Caring for gastrostomy tubes and urinary catheters.
• Administering nebulized treatments and wound vac care.
• Managing CPAP care and additional assigned duties.

Required Experience / Certifications / Licensure

• Proven experience as an LPN, preferably in a Long Term Care setting.
• Current LPN licensure in accordance with state regulations.
• Proof of flu vaccination or a facility-approved application for a medical or religious exception is required prior to the start date.
