5x8.0 shift, flexible schedule with 1-2 evenings until 7 PM.

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patients' needs, goals, and physician direction. This role entails the delegation and direction of all discipline-specific therapy services for assigned patients and ensures the design of functional treatment programs that allow for effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves while maintaining compliance with the organizational Code of Conduct, professional standards, and the mission and core values of the organization.

Responsibilities

• Complete comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document medical records in a clear, concise, and timely manner, ensuring compliance with state and federal guidelines.
• Provide quality care under signed, valid physician orders and within insurance approval.
• Participate in the student program as assigned, offering a positive learning environment.
• Create an individualized professional development plan and pursue ongoing professional education.
• Adhere to discipline-specific State Practice Act and maintain professional licensure.
• Demonstrate awareness of performance improvement initiatives and provide feedback for enhanced outcomes.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license for practice as a Physical Therapist.
• Previous experience with the agency or Select Medical is highly desired.
