Assignment Duration
This is a 13-week contract position.
Schedule
5x8.0 shifts with flexibility, 40 hours, including 1-2 evenings until 7 PM.
Job Description
The Outpatient Physical Therapist is responsible for evaluating all assigned patients, developing, implementing, and modifying treatment plans in alignment with patient needs, goals, and physician directions. This role involves delegating and directing discipline-specific therapy services, ensuring the design of functional treatment programs that facilitate effective returns to community activities. The therapist serves as an educator for patients, families, students, and colleagues, maintaining high professional standards and adherence to the organization’s Core Values, Code of Conduct, and safety protocols.
Responsibilities
- Complete comprehensive evaluations of assigned patients, developing appropriate treatment plans and goals.
- Document medical records clearly, concisely, accurately, and in a timely manner, ensuring compliance with relevant guidelines.
- Participate in student programs as needed, fostering a positive educational environment and supervising students effectively.
- Create individualized professional development plans and pursue continued education and professional growth.
- Adhere to the discipline-specific State Practice Act and maintain active professional licensure.
- Understand and implement organizational performance improvement methodologies to enhance service quality.
Required Experience / Certifications / Licensure
- Degree in Physical Therapy from an accredited institution.
- Minimum of 1 year of outpatient clinical experience.
- CPR Certification.
- Active and applicable state licensure for practicing as a Physical Therapist.
