Hospice Registered Nurse (RN)

Job Description

We are seeking an experienced Hospice Registered Nurse (RN) to join our central Vermont hospice home care agency. The ideal candidate will have a strong background in hospice case management and experience providing hospice care in a home setting. This position requires prior experience with rural travel and familiarity with McKesson/Netsmart EMR systems.

Responsibilities

• Provide high-quality hospice care to patients in a home setting.
• Manage hospice cases effectively, ensuring compliance with care plans.
• Coordinate with interdisciplinary teams to deliver comprehensive patient care.
• Document patient care activities accurately using McKesson/Netsmart EMR.

Required Experience / Certifications / Licensure

• Active Registered Nurse (RN) license.
• Experience with hospice case management.
• Experience in providing hospice care in home settings.
• Preferred experience with rural travel.
• Preferred knowledge of McKesson/Netsmart EMR systems.

Schedule

This position is scheduled from Monday to Friday.
