Licensed Practical Nurse (LPN)

Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality care for adult and geriatric patients. The LPN will work under the supervision of a Registered Nurse, contributing to the assessment, planning, and implementation of patient care plans. The position requires a commitment to patient safety, effective communication, and collaboration within an interdisciplinary team.

Responsibilities

• Collect and analyze data on residents' physical and psychosocial conditions, communicating findings to the RN and team.
• Develop and implement culturally relevant care plans, involving residents and families in the process.
• Assess resident outcomes and recommend revisions to care plans, ensuring all aspects of care are addressed.
• Maintain knowledge and competency in nursing practice through continuing education and participation in inservice programs.
• Participate in the mentoring or orientation of new staff and provide leadership under RN supervision.

Required Experience / Certifications / Licensure

• Active New York LPN Licensure
• BLS Certification
• Minimum of 1 year experience as an LPN, preferably in a long-term care facility
• Availability to work a minimum of 24 hours within a 4-week period, including 2 weekend shifts

Schedule

This is a part-time position requiring a minimum of 24 hours or 3 shifts within a 4-week period.
