Job Description

We are seeking a dedicated healthcare professional to join our team. The candidate will be responsible for providing high-quality patient care, working collaboratively within a multidisciplinary team, and maintaining a professional and ethical standard in all interactions.

Responsibilities

• Deliver exceptional patient care in accordance with established guidelines.
• Collaborate with healthcare team members to provide integrated care plans.
• Document patient health information accurately and timely.
• Participate in case management and quality improvement initiatives.

Required Experience / Certifications / Licensure

• Active License/Certificate Verification required.
• Completion of health screenings and immunization records necessary for patient interaction.
• Trinity Specialty Specific Competency Testing is mandatory.
• Mandatory background check and education verification.
