This is a 13-week contract position.

Schedule

Monday to Friday, 8 AM to 5 PM, with occasional back-up for weekends and on-call hours.

Job Description

Responsibilities

• Lead the interdisciplinary team in providing personalized patient care.
• Help patients identify their personal goals of care and manage their symptoms effectively.
• Utilize strong clinical skills and critical thinking in assessments.
• Provide education and support to caregivers for managing their loved ones' care.
• Deliver care in various settings including private homes, nursing homes, and assisted living facilities.

Required Experience / Certifications / Licensure

• Valid Registered Nurse (RN) license.
• Strong clinical skills and experience in symptom management.
