Schedule

Monday - Friday, 8 AM - 5 PM, with occasional weekend and on-call duties.

Job Description

Responsibilities

• Provide compassionate care to hospice patients and their families.
• Utilize strong clinical skills to assess patient needs and manage symptoms effectively.
• Work collaboratively with an interdisciplinary team for patient-centered care.
• Teach patients and caregivers about personal goals of care and confidence in caregiving.
• Manage an assigned caseload of patients, ensuring excellence in assessment and critical thinking.
• Provide care in various settings, including private homes, nursing homes, and assisted living facilities.

Required Experience / Certifications / Licensure

• Active RN license.
• Experience in hospice or palliative care preferred.
• Strong clinical assessment and critical thinking skills.
