Job Description

We are seeking a dedicated LPN specializing in rehabilitation nursing to join our team. The LPN will provide essential care and support to adult patients recovering from various conditions, including strokes, brain or spinal cord injuries, amputations, and complex orthopedic injuries.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor patient vitals and report significant changes to the nursing team.
• Assist in daily living activities and rehabilitation exercises.
• Communicate effectively with patients and their families regarding care and progress.
• Maintain accurate patient records and documentation using EHR systems.

Required Experience / Certifications / Licensure

• LPN certification
• Basic Life Support (BLS) certification
• Valid state licensure as a Licensed Practical Nurse
• Experience in floor nursing preferred; rehab nursing experience is a plus
