Shift: 8:00 AM - 5:00 PM
Weekend: Every other weekend with on-call rotation overnight.

Job Description

We are seeking an experienced Field Case Manager to join our Hospice team. The ideal candidate will manage a caseload of patients, providing quality care and support to those in need. This position requires strong clinical skills and the ability to work collaboratively within a team.

Responsibilities

• Case manage 14-18 patients, seeing 5-6 patients daily.
• Conduct admissions, recertifications, and discharge processes.
• Collaborate with healthcare team members to deliver comprehensive care.
• Utilize Epic EMR for documentation and patient management.
• Participate in on-call shifts as scheduled, providing after-hours support.

Required Experience / Certifications / Licensure

• Minimum of 1 year of home health or hospice experience required.
• Current RN licensure in the state of Maine.
• BLS certification required.
• Experience with admissions and in-home infusion pumps preferred.
