Day shift, 8-hour shifts, with 40 hours guaranteed weekly. Rotating weekends as needed (2-3 per month).

Job Description

The Home Health RN will provide excellent patient care by participating in the collaboration, development, implementation, revision, and reporting of the case management program. This role acts as a liaison between patients, families, nurses, physicians, the multidisciplinary team, and healthcare benefactors to optimize outcomes. Key responsibilities include leading coordination of patient care, monitoring the appropriateness and timeliness of care, and discussing treatment plans with physicians.

Responsibilities

• Coordinate patient care with multidisciplinary team members, focusing on complex cases such as chronic pain and substance abuse patients.
• Ensure interdisciplinary care plans align with the patient’s clinical course and care needs.
• Monitor diagnostic testing, treatments, procedures, and resource utilization.
• Facilitate patient transfers to tertiary centers, ensuring continuity of treatment and services.
• Conduct focus studies on frequent emergency department visitors and analyze admission data.
• Provide oversight and collect essential data for regulatory and accreditation compliance.

Required Experience / Certifications / Licensure

• 2+ years of experience in a Medicare-certified home health agency.
• Active and clear driver's license and car insurance.
• Hawaii RN license (must obtain within 30 days of start). Temporary license takes 15-20 business days to process.
• Experience with Homecare Home Base and OASIS documentation.
• COVID and FLU vaccine are required (declinations not accepted).
• Bachelor's degree and previous case manager experience are preferred.
