This is a 13-week contract position.

Schedule

Monday to Friday, day shifts; 5 × 8 hour shifts, totaling 40 hours per week.

Location

San Fernando Valley and Santa Clarita, CA (Coverage area includes a 60-mile radius from the facility).

Job Description

Seeking a Registered Nurse (RN) with recent home health experience to provide quality patient care within a designated service area. The RN will be responsible for delivering skilled nursing services, including assessments and interventions, in compliance with established protocols.

Responsibilities

• Conduct initial assessments and ongoing evaluations of patient health status.
• Implement and manage individualized patient care plans.
• Perform OASIS assessments and documentation as required.
• Coordinate discharge planning and recertification visits.
• Provide education and support to patients and their families.
• Maintain accurate and timely records of patient visits and care provided.
• Meet productivity expectations of 28-32 points per week.

Required Experience / Certifications / Licensure

• Active RN license in California.
• Previous home health experience within the last year.
• Experience with HomeCare HomeBase (HCHB) is required; other candidates may be considered at a reduced rate if willing to receive training.
• Knowledge of OASIS and home health documentation standards.
