Monday to Friday, day shifts; 8-hour shifts, 40 hours per week.

Job Description

We are seeking an experienced Registered Nurse for Home Health to provide quality patient care. The position involves covering a designated area and may require assisting in other territories within a 60-mile radius. The ideal candidate will possess a strong understanding of home health care and meet productivity expectations.

Responsibilities

• Conduct comprehensive assessments, including SOC (Start of Care), ROC (Resumption of Care), discharge OASIS, recertification, and routine visits.
• Maintain RN productivity expectations of 28 - 32 points per week.
• Document patient care in HomeCare HomeBase; training may be available for candidates without HCHB experience for the first two weeks.
• Ensure adherence to company policies and procedures regarding patient care and documentation.
• Communicate effectively with patients, families, and interdisciplinary team members to ensure high-quality care.

Required Experience / Certifications / Licensure

• Active RN license.
• Recent home health experience is required.
• Experience with HomeCare HomeBase is strongly preferred.
• Ability to work independently and manage time effectively.
