Monday-Friday, 8:00 AM - 4:30 PM with possible rotation of 1 weekend day per month.

Job Description

The Home Health Physical Therapist will provide care to patients in their homes or nursing facilities, specifically focusing on those with terminal illnesses such as cancer, heart disease, lung disease, and dementia. The therapist will cover a designated area including Doddridge, Lewis, Upshur, Barbour, and the southern portion of Harrison County.

Responsibilities

• Conduct 5-6 patient visits per day.
• Utilize the EPIC electronic medical record system for charting.
• Collaborate with other healthcare professionals in providing patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of Home Health experience required.
• Current Basic Life Support (BLS) certification (AHA).
• Valid West Virginia Physical Therapy license.
