Job Description

Seeking a Case Manager RN with acute care experience to support discharge planning and care coordination. Must be familiar with Epic and have experience working in a complex healthcare setting. This position requires weekend shifts every other weekend or four weekend days within a four-week schedule.

Responsibilities

• Coordinate patient care and discharge planning.
• Utilize Epic for documentation and management of patient care.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
• Adhere to HIPAA guidelines and CMS regulations.
• Assess patient needs and develop customized care plans.

Required Experience / Certifications / Licensure

• Active Alaska RN license required.
• BLS certification preferred.
• ACM certification preferred but not mandatory.
• A minimum of 1 year of acute care experience.
• Previous experience with Epic charting system is required.
