Day shift, 8-hour shifts, 5 days per week from 08:00 to 17:30.

Job Description

The position entails providing therapeutic care in a skilled nursing facility, focusing on geriatric and neurology disciplines. The ideal candidate will possess the necessary skills to assess, plan, implement, and evaluate patient care in a multidisciplinary environment.

Responsibilities

• Perform patient assessments and document findings.
• Develop and implement individualized care plans.
• Conduct evaluations and reassessments according to established protocols.
• Monitor vital signs and respond to patient needs effectively.
• Educate patients and families on care plans and safety measures.
• Utilize adaptive devices and therapeutic equipment to assist patients.

Required Experience / Certifications / Licensure

• Minimum of 1 year of previous charge experience.
• Experience with PointClickCare charting system required.
• Current California physical therapy board license and BLS certification.
• Proficiency in geriatrics, neurology, and skilled nursing practices.
