M-F, 8:00 AM - 5:00 PM, with rotating weekends.

Job Description

The Case Management position involves coordinating patient care and ensuring effective discharge planning. The ideal candidate will engage in care coordination and utilize their skills in utilization review and Interqual criteria.

Responsibilities

• Conduct discharge planning for patients.
• Coordinate care and services while maintaining effective communication with healthcare professionals.

Required Experience / Certifications / Licensure

• Must have Interqual and utilization review experience.
• Experience in discharge planning is required.
• BLS certification is required.
