Schedule

Day shift, 5x8-hour schedule (08:00 - 16:30). Please note that this facility cannot accommodate 4x10 or 3x12 shifts.

Job Description

We are seeking a dedicated Home Health Physical Therapist to provide one-on-one care to patients in their homes. This role requires a skilled professional who can manage a varied caseload while ensuring high-quality patient care standards.

Responsibilities

• Visit up to 6 patients per day, ensuring productivity standards are met.
• Conduct patient evaluations, develop care plans, and implement treatment.
• Educate patients and their families on home exercise programs and rehabilitation techniques.
• Collaborate with a multidisciplinary team to provide comprehensive care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• Current BCLS certification.
• Oasis experience is necessary; please specify where you obtained this experience.
• Reliable transportation is a must for travel between patient homes.
