Job Description

Seeking an experienced Home Health Physical Therapist to provide quality care to patients in Medford, OR and surrounding areas. The role involves visiting patients in their homes and providing physical therapy services, ensuring that each patient receives personalized and effective rehabilitation care.

Responsibilities

• Deliver one-on-one physical therapy to home health patients.
• Manage a caseload of up to 6 patients per day.
• Document patient progress using the EPIC/Oasis charting system.
• Coordinate with interdisciplinary teams to optimize patient care.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• Current BCLS certification required.
• Experience with Oasis documentation is necessary.
• Travelers must have reliable transportation for patient visits.
