Job Description

We are seeking a dedicated and experienced Physical Therapist to join our outpatient clinic team. This role requires a clinical review to ensure a strong fit within our guidelines. The selected candidate will work with a variety of patients, including those recovering from surgeries and managing chronic conditions.

Responsibilities

• Provide assessment and treatment to patients with various physical conditions.
• Develop and implement personalized rehabilitation plans.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families on care plans, exercises, and lifestyle modifications.
• Document patient care and treatment outcomes accurately using EPIC system.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in a relevant clinical setting.
• Current PT License in PA or a COMPACT state.
• Experience with EPIC charting system is required.
• ACLS certification is preferred.
• Ability to provide care for COVID patients may be required.
